Lost / Stolen US Passport
Information for replacing a lost/stolen passport
 

 

 

1. Report the lost passport to a police station or police box nearest to where you lost
your passport. An affidavit of a lost/stolen passport must also be completed at the US
Embassy.

2. Obtain an Entry/Exit Certificate from Sejong Branch of the Seoul Immigration Office.
Address : Junksun Hyundai Building. Room 501, Sejong-Ro, Chongno-Gu, Seoul
Directions : By subway, take the purple line (#5) to Kwanghwamun station.
It is located near the US Embassy, one block west of Kwanghwamun Gate on the south side of the street.

3. Go to the US Embassy in person.
Address : 82, Sejongno, Chongno-Gu, Seoul, S. Korea
Directions : By subway, take the purple line #5 to Kwanghwamun station. Exit #5
Hours of operation :  Monday - Friday (9AM - 4PM)

4. The following items are required for renewing a passport:
A. Form DSP-11, Application for a US passport
B. Proof of US citizenship : US birth certificate, naturalization certificate, Consular of
    Birth Abroad or previous US Passport
C. Proof of Identity : (Example: driver's license, military ID, etc.)
D. Three passport-sized photos, size 2" x 2" with a light background
    (no instant photos)
E. Application fee : $40.00 US (16 years and below), $60.00 (Age 16 and above.) Won
    accepted. No personal checks accepted.

Information in replacing a lost / stolen passport

1. Report the lost passport to a police station or police box nearest to where you lost you passport. An affidavit of a lost/stolen passport must also be completed at the Canadian Embassy.

2. Go to the Canadian Embassy in person.
Address : 10th Floor, Kolon Building, 45 Mugyo-Dong, Seoul, S. Korea
              Hours of operation: Monday - Friday (8:30 AM - 11 AM & 2PM - 4PM)    

3. The following items are required for renewing a passport :
A. Passport application form (available at the reception desk)
B. Proof of Canadian citizenship: birth certificate or Canadian citizenship certificate or
    previous passport
C. Proof of Identity : (Example: driver's license, SIN, health card, alien registration
    card, etc.)
D. Three passport-sized photos (5 x 7 cm) (no instant photos)
E. Application fee : $60.00 CDN (if you have a guarantor) or $110 CDN (if you do not
    have a guarantor)

4. It is recommended that you renew your passport at least 6 months before it expires. Most Asian countries require that you have a passport valid for more than 6 months before allowing entry.
